Disease Management Programs

Chronic disease is a major driver of rising health care costs as people with chronic conditions typically use more health care services and resources. As the prevalence & associated costs of chronic disease continue to grow, management of such conditions becomes a pressing issue for those paying the costs of care.

Care for the chronically ill presents an opportunity for improved health outcomes through disease management programs. Disease management programs use proactive and preventative approaches to provide better care for patients with specific chronic conditions, reduce complications and utilization of expensive hospital resources.

Manzil Disease Management Programs focus efforts on improving overall health for patients with chronic disease through an integrated care approach. Manzil’s unique strategy features broad-based educational programs, care coordination and outcome evaluation.

All program efforts are based on Manzil's underlying premise that individuals can make informed choices about their own health and medical care if they are provided information about disease etiology, complications and self-care management. Manzil disease management programs help participants understand the factors that impact health status and support them in making changes toward improved health outcomes.

Manzil does so through a range of services, including:

  • Phone-based health coaching by specially trained registered nurses. The nurses provide health and medical information and help participants develop a plan to self-manage their condition. Additional educational effortsinclude emailing condition-specific educational material.
  • Routine reporting/feedback to treating physician to ensure continuity of care & encourage adherence to physician’s instructions.
  • Manzil disease management programs promote:

    - Improved care for individuals with chronic diseases

    - Improved course and outcome of disease

    - Reduced need for acute medical intervention

    - Improved participant quality of life

Measurable impact is what it's all about. With Manzil’sdisease management programs, our nurses can impact outcomes for chronically ill patients while containing cost.

    Diabetes Management Program

In 2015, the International Diabetes Federation (IDF) estimated that 15% - 20% of people living in the United Arab Emirates have diabetes. This alarmingly high prevalence calls for an urgent action. Uncontrolled diabetes can lead to serious complications which results in reduced quality of life and premature mortality. Diabetes management is one strategy for reducing this burden.

Manzil Diabetes Management Program provides education and support to patients with diabetes to help reduce short- and long-term complications. Our comprehensive program relies on evidence-based guidelines to help improve patients’ outcomes and quality of life.

Diabetes is a lifestyle disease. Patients living with diabetes are required to make informed decisions about food, activity & medication. In order to achieve optimal diabetes control, patients have to learn to be self-managers and master self-care skills.

Our program offers in-depth education from registered nurses about various topics including:

    - Diabetes

    - Nutrition & physical activity

    - Self-monitoring of blood glucose (SMBG)

    - Blood glucose, blood pressure & cholesterol targets

    - HbA1c monitoring

    - Foot exams

    - Annual exams/tests

Educational material is given to participants to reinforce the importance of self-management & healthy lifestyle choices. When a participant is identified as high risk, they are enrolled in our tele-monitoring program to facilitate the achievement of individualized treatment targets and outcomes.

To that end, we will measure impact in these key areas:

    - Clinical outcomes

o Physical (BMI, blood pressure, weight, retinal exams)

o Biochemical (HbA1c, blood glucose, lipids, kidney function)

    o Presence/absence of long-term complications

    - Cost effectiveness

o Emergency room visits & hospital admissions

    - Self-management &application of knowledge

o Patient knowledge of disease (complications, comorbidities, etc.)

    o Practical skills (ex: SMBG)

    o Medication adherence

o Active participation in care plan
